Kettering Health Hamilton
Identification and assessment of pts and families who have social, psychological and/or environmental needs related to the impact of cancer diagnosis, treatment, or side effects. Essential duties include, but not limited to, interviewing pts and families, developing plans for intervention addressing agreed upon priorities, synchronizing appointments, referral of pts to needed community resources, and assisting pts/families in improving or restoring their capacity for social functioning. Use a proactive approach to facilitate pt care through Staff Huddle and collaboration with the care team. Ensures psychosocial needs are met and understood . Other duties as assigned.
Minimum Education
Bachelor's degree in Social Work required for SW I (117711).
Required Licenses
[Ohio, United States] Social Worker
Ohio Social Work license required. OSW-C (oncology social worker- certification) preferred.
Minimum Work Experience
A minimum of 2 years' experience in acute care and/or oncology setting preferred.
Required Skills
Clinical experience in health care setting and/or community agencies.
Awareness of developmental needs over the age span of adolescent to elder required.
Work well in interdisciplinary and independent settings.
Communicate verbally and through the use of writing skills.
Coordinate work effectively with other hospital disciplines and locations.
Recognize and understand the dynamics of individuals from diverse populations.
Solve problems through the analysis of information.
Network with community agencies.
Provide education to the community and/or medical setting regarding social issues.
Experienced in basic computer skills and electronic medical records.
