Job Summary

 

Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. The Diabetes Educator Dietitian (RD CDCES) is accountable for his/her own professional development and practice within the scope of care defined by law. Utilizing organizational resources, the RD CDCES acts as a patient advocate for the delivery of safe care.  Provides diabetes self-management training (DSMT) and medical nutrition therapy (MNT) to patients with Type 1, Type 2, or Gestational Diabetes in the specialty physician practice setting. Participates in the interdisciplinary care of patients including assessing and monitoring the diabetes management status, nutritional status, developing and implementing individualized care plans, and educating on diabetes self-management, nutritional interventions, therapeutic diets and nutritional needs. Works collaboratively with the interdisciplinary medical practice staff to promote glycemic control for patients.  The Diabetes Educator Dietitian reports to the Administrative Director of Endocrinology.

Responsibilities & Requirements

Job Requirements

  • Education
    • Bachelor or Masters degree from Didactic Program in Dietetics, Nutrition or related field. Dietetic Internship or Pre-professional Practice Program accredited by the Commission on Accreditation/Approval for Dietetics Education of the Academy of Nutrition and Dietetics.

 

  • Work Experience
    • 3 years of clinical nutrition experience preferred.

       Previous experience in diabetes self-management education and support (DSMES) or in a DSMT accredited outpatient program preferred.

 

  •  Licenses, Certifications and Registrations
    • Registered Dietitian with the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics (AND)

      State of Ohio Licensure  as Registered Dietitian (prior to first day of employment)

      Certified Diabetes Care and Education Specialist with CBDCE is preferred.  If not, must be willing to complete criteria for certification and pass the board certification examination within 18 months of hire.

 

Essential Functions

  • Provides appropriate DSMT and/or MNT for referred patients, including adherence to KH/KHMG/department protocols and policies.

  • Effectively utilizes standardized Nutrition Care Processes in providing medical nutrition therapy (MNT) to patients. Accurately assesses, prescribes and maintains nutritional interventions for patient care. Provides direct diabetes self-management care and/or nutrition consultation using in-depth clinical knowledge and skills in area of Diabetes according to the current ADA National Medical Standards for Diabetes and ADA Standards for Diabetes Self-Management Education and Support (DSMES).  Adheres to the requirements and standards for ADA Recognized DSMT programs. Works collaboratively with RN Diabetes Educator in providing DSMT services. Consults with Endocrinology physicians, APPS and multidisciplinary team as appropriate to develop, implement and evaluate care for patients and families.

  • Educates patients and/or families on self-management skills to promote optimal level of chronic disease management and wellness. Utilizes motivational interviewing to provide positive behavior change.

  • Communicates effectively with physicians, nursing and interdisciplinary team to coordinate patient care. Appropriately documents in medical records with clinically relevant, concise, appropriate notes and correct use of approved abbreviations.

  • Effectively communicates patient nutritional and diabetes care needs through electronic medical record systems and department process. Conducts effective nutrition counseling/education classes in person or via telehealth phone/video in individual appointments and/or group settings. Participates in planning, collection, implementation and study of medical practice and DSMT program performance improvement activities.

  • Educates residents, interns or students and other healthcare providers and members of the community; serves as an effective role model.

  • Receptive to change, learns and adapts clinical practice to incorporate cutting edge diabetes and nutrition care, as directed by medical director, administrative director, KHMG leadership and KHMG patient care, safety and quality councils. Actively participates in professional continuing education activities and maintains all required license, registration, and certifications.

 

New Hire/Annual Competencies  

  • Documentation in Epic electronic medical record system and timely closing of charts, entering billing/coding appropriately and accurately of services provided.

  • Basic Life Support (BLS)

  • All required KH annual competencies

  • Completion of DSMT recognized program required 15 continuing education hours if non-certified educator (CDCES)

Skills

  • Strong verbal and written communication skills. Computer literacy in electronic medical records and business software applications preferred. Able to manage time efficiently and independently.

    Flexibility in recognizing different patient needs and adapting to using different communication approaches to best serve the patient.

    Adept at providing respectful, culturally competent care across a diverse patient population.

    Skilled in technology systems including telehealth, diabetes, nutrition and other related smart phone/device apps, blood glucose monitors, continuous glucose monitoring devices and reports, insulin pump technology and other insulin delivery device systems.
