Department Overview

The Oncology Social Worker supports patients who have complex psychosocial and discharge planning needs. The Social Worker coordinates care and services of patients across the continuum of care and throughout cancer treatment. The Social Worker conducts a detailed psychosocial assessment, provides linkages with community resources and community counseling. Additionally, offers crisis interventions to patient and families with psychosocial needs and understands the unique challenges faced by patient with a cancer diagnosis.

Responsibilities & Requirements

Ability to provide extended patient and family therapy related to cancer treatment.
Assist patients in navigating the healthcare system and accessing necessary resources.
Understanding of and ability to manage patients with high distress screen/score and complex mental, physical, emotional, and financial health issues.
Participates in or assists with grant writing and maintenance of grant funding related to program support services.
Provides support to food pantry including fundraising and food collection/distribution.
Maintains familiarity with a wide range of transportation resources including managing grant funded resources and services; able to triage transportation options to maximize organizational resources.
Ability to provide in-depth assistance with financial resources, insurance, disability, and work-related issues over the course of cancer treatment.
Participates in the provision of education to the health setting and community regarding oncology specific social issues.
Participates in leading support groups for a variety of oncologic diseases and age groups.
Provide education on and assist with Advanced Directives

Preferred Qualifications

Minimum Education

Bachelor’s degree in social work - required

 

Minimum Work Experience
Clinical experience in health care settings and/or community agencies - preferred
Outpatient health care experience - preferred
Experience with oncology population - preferred.

 

Required Licenses
Licensed Clinical Social Worker
Licensed Social Worker in the State of Ohio – required.
OSW-C (certified Oncology Social Worker) preferred
