Do you want to work with a team of highly engaged healthcare providers where you can make a direct impact on patient care for our community? If so, this position is for you!


APP will have a pivotal role in the advance heart failure program helping to oversee, manage and coordinate care of these high risk patients. Patience, empathy and attention to detail will be key requirements. Diagnose and treat patients based on physical assessment and evaluation of data. Develop a comprehensive plan of treatment for the heart failure patient in a team approach. Collaborate and co-manage patients with HF physician expert. Co-manage inpatient and outpatients in the Kettering and Centerville area.  Consult dietician, social work, pharmacy, home health, cardiac rehab, sleep labs and other health team members to coordinate resources necessary for optimal patient outcomes.

Responsibilities & Requirements

Educational Requirements:

  • Masters Degree required - National certified APRN- ACNP Ohio Licensure
  • ACLS certified
  • 3 years minimum of cardiac experience required

  • No new Graduates
